How to Write a Compounded Prescription

Prescription Guidelines

  1. Prescriptions should begin with the phrase “Compounded Medication.”
  2. Include the generic name of the active ingredient(s), strength, dosage form, quantity, and directions for use.
  3. Prescriptions can be transmitted to the pharmacy via ePrescribing (including Blend Scripts), fax, or phone authorization to compound.

Now Offering Blend Scripts: An Easier Way to Prescribe
