How to Write a Compounded Prescription

- Prescriptions should begin with the phrase “Compounded Medication”
- Prescriptions should contain the generic name of the active ingredient(s), dose, dosage form, quantity, and directions for use.

Prescription can be transmitted to the pharmacy via escribe containing authorization to compound, fax or phone call.
